Fillers (injectables) are suitable for naturally filling up the superficial and deeper wrinkles in the skin, enlarging or strengthening the lips, restoring or adding volume under the eyes, and improving the facial contours.
The safest and most reliable modern fillers are hyaluronic acid, a substance that naturally occurs in the body which hydrates out skin and adds volume to it.

What are fillers?

What Botox and fillers have in common is that they are both used to reduce wrinkles. Both injectables can also be used after each other. Furthermore, the efficacy of both products is temporary, which means they should be applied again after a certain time.

The treatment

During your first visit to the clinic, prior to a treatment, there is always a free consultation.

During the treatment, the filler is injected into the skin with a thin needle. Thanks to the anaesthetic in the gel (Juvéderm contains lidocaine), the injections are barely noticeable. The duration of the treatment depends on the type of treatment.

The result of the treatment is immediately visible. After a treatment, the volume in the treated area(s) has been restored and the skin looks smoother.

Injectables can be divided into two groups:

1. Fillers (hyaluronic acids including Restylane, Juvéderm, Sculptra and Radiesse)

2. Botox. Botox inhibits the muscle movement that causes wrinkles. Botox is mainly used to remove crow’s feet, the frown lines, and forehead wrinkles.

What are hyaluronic acid fillers?

What Botox and fillers have in common is that they are both used to reduce wrinkles. Both injectables can also be used after each other. Furthermore, the efficacy of both products is temporary, which means they should be applied again after a certain time.

Hyaluronic acid is one of the building blocks of the body. It not only occurs in the skin, but also in our connective tissue, nerve tissue, and epithelial tissue. Hyaluronic acid is a sugar, which means it also falls under glucose building blocks. Hyaluronic acid improves the flexibility and resilience of the skin. For filling up wrinkles and (temporarily) replenishing volume, we mainly use Restylane and Juvéderm.

How does a hyaluronic acid filler work?

Hyaluronic acid is an endogenous substance that is naturally broken down in one or two days, so the body continuously produces new hyaluronic acid. As we age, this process slows down, which causes the skin to be less hydrated and the volume to decrease. This means the skin becomes weaker and wrinkles may occur.
Because the hyaluronic acid filler (such as Juvéderm and Restylane) attracts and can hold moisture, it fills up the skin from the inside. With these pads, we can replenish a volume deficit, hydrate the skin, and fill out imperfections. It also stimulates the production of natural collagen.

The effect of the filler treatment lasts about 6 to 18 months and is then fully broken down by the body. If desired, the product can even be dissolved. We use hyaluronidase for this.

Filler types and use

At Kliniek Dokter Frodo, we only work with non-permanent fillers which are broken down by the body over time. We mainly work with biodegradable fillers based on hyaluronic acid, namely Juvéderm and Restylane.

Juvéderm filler has a soft consistency and gives a long-lasting and natural result. For the treatment of puffiness under the eyes and lips, we use Juvéderm Volbella because of the natural shape and the property that it causes almost no swelling. We usually treat other areas with Juvéderm Voluma. Voluma provides a lot of ‘lift’ and corrects volume loss near the cheeks, cheekbones, temples, nasolabial fold, corners of the mouth, chin, and the jawline.

Restylane hyaluronic acid filler. Restylane is a slightly harder form of hyaluronic acid than Juvéderm and therefore ideal for filling up fine lines and larger volumes. With Restylane, the size of the hyaluronic acid particles varies. The smaller ones (Restylane Vital) are more suitable for fine lines, and the larger ones (Restylane Perlane) for deeper wrinkles.

Radiesse fillers voor liquid facelift of oppervlakkige collageenvorming

Radiesse filler is a long-term, but non-permanent filler. It is a gel with calcium hydroxylapatite (CaHa) particles. CaHa particles occur naturally in everyone’s bodies. The gel surrounding the CaHa particles dissolves in the body within a few weeks and is replaced by the body’s own tissue. The CaHa particles are then broken down naturally over approximately two years. After that, the filler is no longer visible.

Sculptra fillers

Sculptra filler is a 100% biodegradable polylactic acid, a synthetic and hypoallergenic material. Sculptra stimulates the production of the own collagen in the skin. The polylactic acid is broken down over a period of several months, while new collagen tissue is produced at the same time. Even once Sculptra has fully disappeared from the body, the effect of the newly formed collagen tissue remains. Sculptra is special because it is not only suitable for reducing the effects of skin aging, but also deals with the cause of wrinkles and lines: collagen and bone loss. Sculptra restores volume by, where needed, stimulating the body’s own production of collagen to reduce lines and wrinkles and cause your youthful appearance to return in a natural way.

Kliniek Dokter Frodo does not work with permanent fillers

Permanent fillers consist of substances which do not occur naturally in the body and cannot be broken down by the body. Permanent fillers have been banned since this year, except for medical-reconstructive purposes.

Potential risks of permanent fillers (which can even occur after years) are infections, hard lumps, and (excessive) capsular contracture around the injected filler. If the skin ages further, or e.g. after severe weight fluctuations, the injected substance can become visible as lumps under the skin. Bad news in the media about fillers is almost always about these substances.

Fillers, Botox and skin improvement

The ideal treatment for certain wrinkles is often a combination of Botox and a filler. By first weakening the muscles with Botulinum Toxin and then strengthening the skin with a filler at the site of the wrinkle, we can achieve an optimal result. To enjoy the results even longer, a programme focused on skin improvement is a valuable addition to a treatment with injectables.

After the treatment

After the treatment, the treated area may be somewhat red and swollen. Use a coolpack to reduce the swelling. It is recommended to also cool at home at some point on the same day. You can do this by using a coolpack or melting ice cubes in a plastic bag, wrapped in a thin towel or washcloth.

Do not expose the treated area to intense heat or intense facial treatments during the first week after treatment.

Risks and side effects of fillers

The most common side effects are local redness and swelling of the skin. There may also be a feeling of numbness.

There is a small risk of bruising. In most cases, the side effects disappear within 2 to 7 days. A possible bruise has no adverse effect on the final result and is often easy to camouflage.

Experiences with fillers

My appearance bothered me. I am generally an energetic person, I love being busy. I want to radiate this. When I’m busy, people often ask me if I shouldn’t take it easier. As if I can’t handle it!’ This lady has had a Liquid Facelift with Restylane. With this treatment, the face is lifted with hyaluronic acid fillers, and there is nu ‘cutting’ involved. ‘I’ve already had so many compliments, I feel fantastic!’ – Marcella (37)

Safety of fillers

There are various fillers on the market. We mainly use hyaluronic acid fillers by Juvéderm and Restylane. Worldwide, over 20 million treatments have been carried out with these fillers, which means there is a wealth of experience.

All products have been rigorously tested in clinical trials in which their safety for cosmetic use has been proven. The fillers remain in place after injection and if used properly, the risk of complications is virtually nil.

Because all fillers in our clinic are biodegradable,  adverse reactions rarely occur. In the unfortunate event that they do, we can dissolve the filler; something that is not possible with permanent fillers. We use hyaluronidase for this.
